The HSE has launched an updated Health Passport App

 
 

We wish to let you know that the HSE has launched an updated Health Passport App. It will support healthcare staff understand the needs of the passport holder, and ensure they have supports required, before any assessment, examination or treatment of the person.

This App includes sections on Critical Information, Personal Information, Communication, Decision Making, Medical History, Food and Drink, and Personal Care. There is an option to email the passport, if required, to a health or social care professional prior to an appointment or admission to the hospital.

All information is saved to the passport holder’s personal device and can be viewed by opening the App and clicking on the relevant tab.

The HSE are currently organising Popups in hospitals where there will be an information desk for any interested parties including staff, individuals, families, etc. Health Passport Pop up Locations and Dates are as follows:

August 15th 2025 Midland Regional Hospital Tullamore- 10am-1pm
August 18th 2025 University Hospital Limerick- 2pm -430pm-OPD Corridor
